Good Faith Estimate notice for Shelby Castile, LMFT

  • Not using insurance

  • Choosing to self-pay for services

  • The anticipated number of sessions (if known)

  • Additional services (coaching, consultation, extended sessions) may vary and will be discussed in advance

4. Disputes

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.
